I want my athlete to complete the bridge the gap program. Are we obligated to complete a certain number of weeks?

No. You are never obligated. If an athlete is feeling better, doing better, and not reporting any lasting issues, then they are free to be done at any time with a 24-hour cancellation notice.

How long do we need to come?

There is always room for improvement. This all really depends on how quickly an athlete can invest into their improvement with full effort and do their homework regularly.

How do I pay?

Elite Sports & Performance Academy (ESPAofAL) is a cash-based clinic. You do not need a doctor’s referral to see Dr. Jazz in some programs nor does ESPAofAL accept insurance. We accept payments by card, cash, and/or Venmo.

Cash-Based!? What’s that?

With cash-based payment, all of our services will be performed by a licensed Doctor of Physical Therapy. Our individualized sessions greatly help educate the client so the TEAM understands all underlying processes which could lead to faster results. With the population of clients we see, most of our clients don’t ever reach their deductible yearly which makes formal PT even more expensive than cash-based. With another percentage of our population, insurance only covers so many visits or they have capped out after an extensive surgical and post-surgical protocol like an ACL reconstruction.

Do I need a doctor’s referral?

No, you don’t need a referral in general. Our physical therapists operate under the Alabama Physical Therapy Practice Act Section 34-24-210.1. (a) Without prescription or referral, a licensed physical therapist may perform an initial evaluation or consultation of a screening nature to determine the need for physical therapy and may perform the physical therapy and other services provided in subdivisions (1) to (5). (4) Related to conditioning or to providing education or activities in a wellness setting for the purpose of injury prevention, reduction of stress, or promotion of fitness. A dentist referral is needed; however, for any TMJ/TMD clients.
